OVERVIEW
The annual dues cycle follows the calendar year. Annual dues are payable January 1 of each year.To access a copy of the 2011 dues payment forms, select the appropriate form below. NOTE: Current and prior years dues payments are not refundable.

2012 Dues Payment Form: Fellows
- Dues sum = $325
- Dues payment includes a subscription to the print version of the Optometry and Vision Science journal

2012 Dues Payment Form: Emeritus Fellows
- In accordance with the Academy's bylaws that were updated during the December 10, 2005 Business Meeting,
- The Board of Directors may grant Emeritus Fellowship to any Fellow who has been a Fellow for at least 30 years and has reached age 70.
- Exception to the December 2005 updated bylaws: The Board of Directors may grant emeritus Fellowship to any Fellow born during 1940-1945 who has been a Fellow for at least 30 years and has reached age 65.
- Dues sum = $80
- An option to subscribe ($62) to the print version of Optometry and Vision Science is included on the form

2012 Dues Payment Form: Senior Fellows
- In accordance with the Academy's bylaws that were updated during the December 10, 2005 Business Meeting, the Board of Directors may grant Senior Fellowship to any Fellow who has reached age 65. Requests for such considerations by the Board of Directors are to be submitted in writing to the Academy office.
- Dues sum = $225
- Dues payment includes a subscription to the print version of the Optometry and Vision Science journal

2012 Dues Payment Form: Honorary Fellow, Life Fellow, Disabled Fellow or Life Emeritus Fellows
- Dues sum = zero

2012 Dues Payment Form: Candidates for Fellowship
- Dues are prorated for up to three years post graduation, based on the year a candidate received his professional degree; they range from $30 to $325.
- Dues payment includes a subscription to the print version of the Optometry and Vision Science journal
