Abstract BACKGROUND: Many Americans were called to serve and sustained physical, mental and ocular casualties during the Korean conflict. The establishments of MASH units increased survival rates. This poster will present a phenomenal case and clinical course of a retained metallic foreign body received some 48 years prior.

CASE REPORT(S). A 100 % disabled 70 year old African-American Korean War veteran presented for a visual analysis at the John Cochran VA Medical Center. His chief complaint was blurred vision OD. His medical history is significant for a shrapnel wound to his left frontal bone, which was repaired with a metal plate when he served in the Korean conflict. Slit lamp evaluation revealed a full thickness stromal corneal scar off the visual axis. There was a horizontal oriented opacity on the anterior capsule and cortex. Within the superior temporal nuclear layers of the lens was a vertically oriented metallic FB, which was rusted and surrounded with a cuff of dense white opacity. Amazingly, the FB twisted within the lens and stopped in the nucleus. His best visual acuity was 20/30 despite the dramatic lens opacification. The intraocular pressure was 12 and 13 mm Hg via Goldmann applanation tonometry. The vitreous and retina was normal.

CONCLUSIONS. Many individuals served our nations cause and sustained many wounds. This lucky veteran almost lost his eye due to shrapnel wound. A metallic foreign body was retained by the lens. Fortunately, the lens is immunologically isolated. No further complication has occurred over a period of 48 years. This dramatic and rare case will be presented and discussed in comparison to other forms of intraocular FB and types of trauma. A digital computerized video will be also available for viewing.
